 Time-series are combined where methods are similar over the time span. In some countries data are insufficient to report. 

 Target groups may vary between countries owing to different methods and data sources; therefore comparisons should be made with caution. 

 The estimates of Czech Republic refer to the age group 18–64. 

 The age range for Malta in 2006 is 12 to 64 years old. 

 The original age range of studies for Finland was 15–54.
